  Requirements for Membership Card

  1. The card is to be used solely by the beneficiary.

  2. The beneficiary must be a registered signatory as follows:

    For Sole Proprietorships:
    • The owner

    For Companies:
    • President
    • Board member
    • General Manager

  3. Two personal photos and a copy of ID.

  4. A lost card must be reported to the Excellent Membership Center in formal writing to issue a replacement card.
